<p>Stinky nose - technically known as ozaena - is a rare form of chronic rhinitis. It is a persistent inflammation of the nasal mucosa that persists over a long period of time and does not resolve on its own like a normal cold. As a result of this permanent irritation, the mucous membrane dries out considerably and produces less secretion. As a result, dead mucosal cells and bacterial residues thicken to form thick, greenish-brown crusts. These crusts release an intense, unpleasant foul-sweet odour.</p>
<p>Important to know: A stinky nose does not develop overnight, but develops slowly over months or even years.</p>

			<p>Recurrent infections of the nose can severely weaken the mucous membrane. In many cases, the bacterium Klebsiella ozaenae is involved. It belongs to the so-called gram-negative germs. This refers to bacteria that have a special cell wall structure and are therefore often more resistant to certain antibiotics.</p>
<p>Klebsiella ozaenae is not uncommon in the nose and throat. However, if the mucous membrane is damaged or dried out, the bacterium can multiply rapidly and exacerbate the inflammation.</p>
<p>Over time, the mucous membrane regresses and the nasal cavity expands. These changes pave the way for new infections - a vicious circle that favours the typical changes of a stinky nose.</p>

<p>In addition, the mucous membrane can lose its protective and barrier function over the course of a lifetime - e.g. due to frequent infections, environmental stress or operations. Metabolic diseases such as diabetes, hormonal changes during the menopause and certain deficiencies can also affect the condition of the nasal mucosa.</p>
<p>Without sufficient moisturisation, fine injuries occur that pave the way for inflammation. In the long term, the tissue retracts and the nasal cavity widens. These changes significantly favour the development of a stinky nose.</p>

<li><strong>Crust formation and dehydration</strong><br />
Thick, foul-smelling crusts form in the nose, which are difficult to remove and feel unpleasant.</li>
<li><strong>Olfactory disorder (anosmia or hyposmia)</strong><br />
Many sufferers lose some or all of their sense of smell. As a result, they often do not even notice the typical odour themselves.</li>
<li><strong>Obstructed nasal breathing</strong><br />
The crusts and the altered mucous membrane make it difficult to breathe through the nose. Some patients are therefore permanently dependent on mouth breathing.</li>

			<h3>How the diagnosis of ozaena works</h3>

			<h3>How a nasal spray works</h3>
<p>A nasal spray works in different ways depending on the type and active ingredient. The most common variants are decongestant, moisturising and anti-allergic nasal sprays:</p>
<h5>Decongestant nasal sprays (alpha sympathomimetics)</h5>
<p>They contain active ingredients such as xylometazoline or oxymetazoline, which can damage the blood vessels in the <a href="https://de.wikipedia.org/wiki/Nasenschleimhaut" target="_blank" rel="noopener">Nasal mucosa</a> narrow. This causes the mucous membrane to swell, the nasal passages become clear again and breathing becomes easier. These sprays take effect within a few minutes and usually last for several hours.</p>
<h5>Moisturising nasal sprays</h5>
<p>These sprays use saline or hypertonic salt solutions to keep the mucous membranes moisturised. This is particularly helpful in dry air or with mild infections. They work purely mechanically by loosening mucus and supporting the natural cleansing function of the nose. As they do not contain any decongestants, you can use them long-term without hesitation.</p>
<h5>Cortisone-containing or anti-allergic nasal sprays</h5>
<p>These sprays are mainly used for allergies or chronic nasal complaints. Preparations containing cortisone reduce inflammation and swelling over a longer period of time, but only take effect after a few days. Anti-allergic nasal sprays often contain antihistamines or mast cell stabilisers, which block allergic reactions directly in the nasal mucosa.</p>
<p>Depending on the symptoms, a different nasal spray may therefore be useful.</p>

			<h3>What happens if the nasal spray doesn't work?</h3>
<p>If a nasal spray no longer works as usual, this can have various causes. There are different reasons depending on the type of spray and the individual symptoms:</p>
<h5>Familiarisation effect with decongestant nasal sprays</h5>
<p>You should only use decongestant nasal sprays for a few days. Prolonged use can lead to your nasal mucous membranes becoming accustomed to the spray and no longer decongest properly without it (<a href="/en/what-are-the-consequences-of-nasal-spray-addiction/">Nasal spray addiction</a>). This can cause drug-induced rhinitis - the nose remains permanently blocked, even if there is no longer a cold.</p>
<h5>Incorrect application</h5>
<p>A nasal spray only develops its full effect when used correctly. Mistakes such as inserting the spray head incorrectly, pulling it up too quickly or failing to inhale briefly while spraying can result in the active ingredient not being optimally distributed.</p>
<h5>Active ingredient too weak or unsuitable</h5>
<p>Not every nasal spray helps with every cause of a blocked nose. For example, a mild seawater spray can moisturise a dry nose, but cannot relieve acute swelling. Conversely, decongestant sprays also do not help against allergic symptoms.</p>

			<p>A chronic <a href="/en/treating-chronic-sinusitis/">Sinusitis</a> is caused by prolonged or recurrent inflammation of the mucous membranes in the pneumatisation spaces. As a result of this inflammation, the mucous membranes remain permanently swollen, which is often accompanied by increased mucus production. As the mucus cannot drain properly, nasal congestion is further exacerbated.</p>
<p>Decongestant nasal sprays can provide short-term relief, but do not resolve the underlying inflammation. Long-term treatment therefore requires the use of nasal sprays containing cortisone and regular nasal rinsing with saline solution. In the case of a bacterial infection, the administration of antibiotics is also useful. In particularly stubborn cases, surgery may be necessary to permanently improve the ventilation of the sinuses.</p>

<p>As conventional nasal sprays cannot alleviate the symptoms in the long term, treatment initially consists of moisturising the nasal mucosa with nourishing nasal sprays or ointments. If the symptoms are severe, surgical reconstruction of the nasal septum may be necessary.</p>

<p>Sinusitis is an inflammation of the mucous membranes in the paranasal sinuses. The sinuses are air-filled cavities in the skull that are connected to the nasal cavity. Their main function is to humidify, warm and filter the air that is inhaled. <p>Snoring causes disturbing noises due to the vibrations of slack tissue in the throat and palate area when breathing in and out during sleep.<br />
The exact causes and mechanisms of snoring are varied and differ from person to person.</p>
<p>The most common factors that play a role in snoring are</p>
<h5>Relaxation of the throat muscles</h5>
<p>During sleep, the muscles in the throat and palate relax. This causes the soft tissues in this area to contract more tightly. When the air flows past these relaxed tissues during inhalation, the characteristic snoring noises are produced.</p>
<h5>Constricted airways</h5>
<p>Some people's airways are naturally narrower. They can also be narrowed due to anatomical conditions such as an enlarged tongue or tonsils. In these cases, the air flows past with more pressure, causing snoring.</p>

<p>Excessive weight can cause an increased accumulation of fatty tissue in the throat and pharynx area. This can further constrict your airways.</p>

In the open technique, we make a small incision on the bridge of the nose and detach the skin from the tip of the nose. We then fold it upwards to allow a clear view of the bony area. The closed technique, which is suitable for smaller corrections, is different. Here, we operate on your nose from the inside via an incision in each nostril.</p>

<p>A curvature of the nasal septum can be congenital or develop in the course of physical development (e.g. as a result of the posterior bony and anterior cartilaginous parts growing at different rates). Sometimes a curvature of the nasal septum can also develop after an accident or a nasal injury, such as an unrecognised nasal fracture.</p>
<p>If the septum is bent, this creates constrictions in the nasal passages. As a result, more air flows into one nostril than the other. As a result, those affected often have difficulty breathing, especially during physical exertion. Due to the lack of air, they often breathe unconsciously through their mouth. The mucous membranes in the throat dry out. This can increase susceptibility to respiratory infections. Other symptoms can also be associated with a deviated septum: migraine-like headaches, snoring and interrupted breathing during sleep, frequent nosebleeds, repeated middle ear infections, odour disorders or even a huffy and nasal-sounding voice.</p>
<p>To straighten the nasal septum surgically, we make a small internal incision on the front edge of the nasal septum. This allows us to detach the mucous membrane from the cartilage and bone. Using special incision techniques, we can dissolve the curvature and thus straighten the septum. After the procedure, you will need to wear special silicone splints that remain in your nose for 4 days.</p>
<p>If at the same time a <a href="/en/turbinate-hyperplasia/">Nasal concha hyperplasia</a> If the nasal turbinates are present, we can also remove them during the same procedure by <a href="/en/laserconchotomy-frankfurt/">Laser conchotomy</a> downsize.</p>

			<h3>Basic knowledge: Structure of the inner and outer nose</h3>
<p>The nose consists of an outer part (nasal framework) and an inner part (nasal cavity). The nasal septum divides this into the right and left nasal cavities. As a rule, both are approximately the same size - unless there is a <a href="/en/consequences-of-a-deviated-nasal-septum/">Curvature of the nasal septum</a> before. In this case, one main cavity is larger than the other. The nasal septum consists mainly of bone, with cartilage at the front. As an important support, it has a decisive influence on the shape and direction of the outer nose (read our blog post on this: <a href="/en/crooked-nose/">Crooked nose: what to do?</a>).</p>
<p>The nose is connected to the outside world via the two nostrils. There are also inner funnel-shaped openings (<a href="https://flexikon.doccheck.com/de/Choane" target="_blank" rel="noopener">Choanen</a>) that connect the two nasal cavities with the nasopharynx.</p>
<p>The paranasal sinuses adjoin the nasal cavity. These are air-containing cavities. Small channels connect them to the nasal cavity. The paranasal sinuses can become inflamed if these small channels are blocked (sinusitis).</p>
<p>Then there are the turbinates, which protrude from the two lateral nasal walls into the nasal cavity in the form of three thin hook-shaped bony lamellae. They are covered with cavernous tissue and mucous membrane.</p>

<p>A nasal septal perforation is a hole in the nasal septum. There are various reasons why this can occur. These include</p>

			<h3>Special case: hole in the nasal septum due to septum piercing</h3>
<p>The so-called septum piercing has been a trend for years. If everything goes well, the thin membrane in front of the cartilaginous part of the nasal septum is pierced with the piercing. However, getting this piercing is not that easy, which is why women and men who want this type of piercing are strongly advised to go to a studio with an excellent reputation where experienced piercers work.</p>
<p>Each nasal septum is individually shaped and often has irregularities. The cartilage can be hit when piercing - this is very painful and can lead to subsequent problems such as bleeding, inflammation of the piercing area, swelling, allergies or the formation of an abscess.</p>
<p>In addition, the hole in the septum - if the piercing is removed and no longer worn - can remain permanently. If it is too large because a piercing with a large diameter has been worn, there is a high risk that it will not grow back on its own. The remaining hole in the septum is then a potential source of infection or similar.</p>

			<h3>Septal perforation: symptoms and consequences</h3>
<p>A hole in the nasal septum does not necessarily lead to symptoms. Some patients experience no symptoms at all. In this case, we discover the hole more by chance during an examination of the nose. However, this is not always the case.</p>
<p>Septal perforation can also lead to unpleasant symptoms. This can be a dry nose with the formation of crusts and plaque. Bacteria can settle on these crusts and cause inflammation, which is accompanied by yellow and sometimes foul-smelling mucus. Frequent nosebleeds or whistling noises when breathing through the nose are also possible. Some sufferers also report <a href="/en/causes-of-pain-in-the-nose/">Pain in the nose</a>. In some cases, the septum hole obstructs nasal breathing by causing the airflow in the nasal septum to collide with the posterior wall of the hole. As a result, the air can no longer be optimally channelled.</p>
<p>If too much supporting and stabilising cartilage substance is lost due to progressive perforation, this can even lead to changes in the external shape of the nose. It then becomes visible and can be very noticeable in some cases.</p>

			<h3>Hole in the nasal septum: how the diagnosis works</h3>
<p>In order to detect a hole in the septum, we carry out an examination of the inner nose. The first step is to use a so-called nasal spreader. This expands the nostrils and holds them open. This allows us to illuminate the inside of the nose with a special lamp and examine the mucous membrane. Next, we perform a rhinoscopy. To do this, we insert a nasal endoscope into the inside of the nose and can thus examine the condition of the nasal mucosa, the nasal cavities, the nasopharynx and the front area of the paranasal sinuses. Of course - and in this case above all - we can examine the nasal septum and determine whether there is a hole in the septum.</p>

			<h3>Treatment of a hole in the septum</h3>
<p>How the septal perforation is treated depends on the history of its development and the symptoms caused by the hole. One possible therapy is the use of special ointments that promote the healing of the mucous membrane. Nasal rinsing with salt water can also be helpful.</p>
<p>If the hole is very large, is in an unfavourable location, leads to restrictions/severe discomfort or to an externally visible deformation of the nose, surgery may be advisable. The aim of septal perforation surgery is to permanently close the opening in the nasal septum. This is possible by moving various mucosal flaps inside the nose. We also remove cartilage tissue from another part of the body (e.g. the ear), which we implant into the nasal septum to close the defect.</p>
<p>It is also possible to perform an aesthetic rhinoplasty at the same time. During this procedure, we can also remove nasal humps, equalise the nasal saddle, correct a malposition of the nasal septum or correct the tip of the nose, for example.</p>

<p>A crooked nose has a laterally displaced course. The nasal axis tilts visibly to the right or left. A curved nasal septum is usually the cause of the deformity.</p>
<p>A crooked nose is often genetic. This means that the curvature of the <a href="https://de.wikipedia.org/wiki/Nasenscheidewand" target="_blank" rel="noopener">Nasal septum</a> is congenital. This makes the nasal axis slanted - which also affects the appearance.</p>
<p>Sometimes a crooked nose only develops in the course of life. Accidents, falls or even violence can affect the structures of the nose and cause it to shift. Above all, it is fractures of the nasal bone that subsequently lead to a crooked-looking nose. In this case, the bones and cartilage have grown together crookedly rather than straight during the healing process.</p>

In women, the growth phase of the nose is completed from the age of 15 to 16, in men from the age of 16 or 17. We do not recommend having a cosmetic rhinoplasty performed before this age. <p>Blood leaks from the nose when the fine vessels in the nasal mucosa, which is heavily supplied with blood, are damaged. If the leaking blood is dark red in colour, it is venous blood. If it is rather light red in colour and spurts out of the nose, it is blood from the arteries. Generally, most haemorrhages occur in the anterior part of the <a href="https://de.wikipedia.org/wiki/Nase" target="_blank" rel="noopener">Nose</a>as several blood vessels converge here. They are usually harmless. The situation is different if the epistaxis originates at the back of the nose. Here, the supplying vessels are not as finely branched as in the front of the nose. A "leak" is therefore larger here.</p>

<p>The cause of the nosebleed is located inside the nose. It can be triggered by the following factors:</p>
<ul>
<li>Blowing your nose vigorously or sneezing can cause small veins in the nose to burst. If the nasal mucosa is irritated (e.g. due to heating air or prolonged use of decongestant <a href="/en/what-are-the-consequences-of-nasal-spray-addiction/">Nasal spray</a>the risk of blood loss is increased.</li>
<li>Epistaxis can also be caused by a blow to the nose or a fall, as well as by a <a href="/en/broken-nose/">Nasal bone fracture</a> are caused.</li>
<li>If the nasal mucosa is injured by a foreign body (e.g. a Lego brick or a marble in children) or by picking the nose, it can also start to bleed.</li>
<li>Patients who suffer from a septal perforation and have a hole in the nasal septum can suffer from a bleeding nose from time to time. Other malformations can also be the cause, such as protruding bony ridges that injure the nasal mucosa. Incidentally, such findings are often accompanied by a <a href="/en/consequences-of-a-deviated-nasal-septum/">Curvature of the nasal septum</a> on.</li>
<li>In rare cases, a tumour in the nasal cavity or paranasal sinuses may be behind severe and recurrent nosebleeds. This can be benign or malignant.</li>

<p>If the bleeding is exceptionally heavy or you can see a visible injury to your nose, you should seek medical advice. <p>If you experience epistaxis frequently, we will carry out certain examinations after taking your medical history. An ultrasound examination can provide additional information. If necessary, we can call in the radiology department and have an MRI or CT scan carried out.
