<p>People who have diabetes are more than twice as likely to suffer hearing loss than healthy people who are not affected by diabetes. Nevertheless, hearing loss is often not considered to be a consequence of diabetes - and is therefore often not diagnosed. Diabetes can also have an impact on the performance of the ears. Especially in the high-frequency range (important for clearly understanding speech), this can lead to limitations. Furthermore, many diabetics react more sensitively to noise and the ears regenerate more slowly after acoustic trauma.</p>
<p>But how can this happen? The crux lies in the cochlea. It is a spiral-shaped component of the inner ear. The cochlea converts sound waves into nerve impulses. Diabetes can lead to a thickening of the blood vessel walls and thus hinder the cochlea in its work. This is considered to be the cause of hearing loss in diabetes. In addition, diabetes can favour the development of a so-called cochleopathy. This is damage to the inner ear which means that sound waves can no longer be converted into nerve impulses.</p>

			<h3>What is tinnitus?</h3>
<p>Tinnitus is a permanent, unpleasant noise in the ear. It sounds like whistling or beeping - some patients also perceive it as hissing, buzzing or ringing. The ringing in the ears can either occur acutely (no longer than three months) or become chronic (longer than three months). A distinction is also made between objective tinnitus - which can also be heard by us during the examination with a stethoscope - and subjective tinnitus. In this case, only you can hear the ringing and buzzing.</p>
<p>The ringing in the ears can be associated with a physical illness. In addition to typical ENT diseases such as middle ear inflammation, otosclerosis or sudden hearing loss, cardiovascular diseases, arteriosclerosis, high blood pressure or diabetes can also be the cause of tinnitus. Tension in the neck and back or temporomandibular joint problems should also not be neglected.</p>
<p>The psyche can also be a possible trigger. Persistent professional and/or personal stress or other psychological problems can cause ringing in the ears as a reaction to anger and overload.</p>

			<h3>What is botulinum?</h3>
<p>The name Botox has become synonymous with various products that work with the active ingredient botulinum toxin - but it is actually a protected brand name of a manufacturer. This is similar to using the term "Tempo" to refer to a paper tissue in general (which is also produced by other companies).</p>
<p><strong>Botulinum works as follows:</strong></p>
<p>In order for a muscle to move, the associated nerve releases a transmitter substance (acetylcholine). The result: the muscle contracts. Botulinum toxin inhibits the release of the transmitter substance. As a result, the muscle can no longer contract (as strongly) and relaxes.</p>
<p>There are seven serotypes of the active ingredient, which differ in their concentration and properties. They can be used not only for treatments in aesthetic medicine (wrinkle treatment/wrinkle smoothing), but also to improve neurological problems, for example. Common areas of application for botulinum are dystonia, i.e. movement disorders in which involuntary and abnormal muscle movements can occur. These include tremor, torticollis, eyelid spasms, spasticity and strabismus. Botox can also provide relief in chronic cases of migraine.</p>
<p>The active ingredient has also proven itself in the treatment of excessive sweating: In hyperhidrosis, botulinum toxin can prevent increased sweating, e.g. under the armpits.</p>

			<h3>Study: Botulinum for tinnitus therapy - injection into the auricle area</h3>
<p>According to American scientists from the California Ear Institute in San Ramon, Botox can also be effective in the treatment of tinnitus. They presented their findings at a meeting of the American Academy of Otolaryngology in New York.</p>
<p>The <a href="https://www.wissenschaft.de/erde-umwelt/botulinumtoxin-gegen-tinnitus/" target="_blank" rel="noopener">Study</a> included 26 tinnitus patients whose ringing in the ears was first precisely classified. For the tinnitus therapy, they injected the substance into the auricle area of one group of patients. The researchers administered a placebo to the other groups. They repeated the injections four months later.</p>
<p>They then asked the study participants about their subjective feelings and any noticeable changes. In some patients to whom they had administered the substance, they noticed a reduction in ringing in the ears.</p>

			<h3>Other approaches to tinnitus therapy with Botox</h3>
<p>If the ringing in the ears is caused by a dysmotion of the soft palate, injections with botulinum can provide relief. This also applies to the so-called gaping tube, a functional disorder of the tube with an insufficient closure between the nasopharyngeal cavity and the tympanic cavity of the middle ear.</p>
<p>As ringing in the ears is also caused by temporomandibular joint problems, it may be worthwhile using botulinum here. Night-time teeth grinding is often the result of overactive chewing muscles. Those affected often suffer from pain in the jaw joint and tension in the cervical spine. As a result, they wake up in the morning with a sore jaw and a tension headache. These symptoms usually improve over the course of the day. This symptomatology is often accompanied by unilateral or bilateral ringing in the ears.</p>
<p>In addition to neural therapy or treatments with acupuncture and chiropractic therapy, botulinum can also be used in such cases. The active ingredient is injected into the chewing muscle. In some cases, this improves the symptoms.</p>

<p>There are several factors that can play a role, such as a circulatory disorder in the inner ear, which is why the term "ear infarction" is also used in this context. Inflammation or viral infections can also lead to sudden hearing loss. In many cases, high levels of psychological stress, as can occur in the case of private or professional stress, can also lead to or contribute to the development of sudden hearing loss. Triggers of a physical nature are also possible. These include, for example, problems with the cervical spine or the surrounding muscles. However, the cause of the hearing loss often remains unclear.</p>

			<h5>How can sudden hearing loss be treated?</h5>
<p>It is important that you have yourself examined by us if you experience the above symptoms. This is the only way we can recognise sudden hearing loss in good time. The chances of recovery are high.</p>
<p>After we have carried out special analyses (<a href="/en/hearing-and-vertigo-diagnostics-frankfurt/">Hearing diagnostics/audiometric testing</a>) have confirmed the diagnosis, we begin treatment. Infusion therapy with cortisone is possible here in order to restore microcirculation in the inner ear. In addition or as an alternative, we offer medication in tablet form. If neither the infusion therapy nor the administration of tablets achieves the desired improvement, the so-called intratympanic cortisone injection can help. This involves anaesthetising the eardrum and injecting cortisone into the middle ear. The advantage here is that we achieve a significantly higher concentration of the medication in the cochlea or inner ear. The side effect rate of this treatment is very low.</p>
<p>We also advise you to avoid exposure to noise and stress as far as possible when treating sudden hearing loss. Massage and physiotherapy can be beneficial. Patients should stop or at least reduce physical exertion and factors that damage circulation, such as smoking.</p>

Tinnitus can also occur with cardiovascular diseases and problems with blood pressure, constant stress and depression. Sometimes viruses and bacteria also enter the ear area via the secretions, which can lead to an inflammation of the middle ear. If the secretions do not drain away on their own within a few days, treatment is required at our practice. We use the so-called tympanostomy tube to drain the secretions.</p>

			<h3>Changes in the inner ear due to Menière's disease</h3>
<p><a href="https://de.wikipedia.org/wiki/Prosper_Meni%C3%A8re" target="_blank" rel="noopener">Prosper Menière</a>, a French physician, described inner ear disease as early as 1861. It was therefore named after him. According to current knowledge, there is still no clear cause for Meniere's disease. Scientists associate various factors with the occurrence of inner ear disease. These include frequent viral infections (e.g. herpes virus), immunological diseases, injuries, hormonal disorders, circulatory disorders and a family predisposition.</p>
<p>The most popular theory is that of changes in the inner ear. It houses the cochlea and the organ of balance, as well as two different fluids: the perilymph and the endolymph. They are in a sensitive balance and characterise the function of the organ. In Meniere's disease, the resorption of the potassium-rich endolymph in the inner ear appears to be disturbed.</p>

			<p>This causes too much fluid to accumulate, which increases the pressure in the inner ear (endolymphatic hydrops). This is probably due to the fact that the inside of the cochlea is lined with a membranous membrane - the Reissner membrane - which tears here and there due to the high pressure. However, the thin cell membrane is equipped with sensory cells for hearing and balance.</p>
<p>It also separates the perilymph and the endolymph. The tears cause the two fluids to mix, which disturbs their balance. Furthermore, the tears lead to a sudden change in the pressure conditions in the ear. This results in a malfunction of the sensory cells. This explains the symptoms of Menière's disease.</p>

Patients have the feeling that their surroundings are spinning around them very quickly - comparable to a ride on a merry-go-round. The vertigo attacks are sometimes even so severe that those affected have to lie down. Rotational vertigo often causes nausea, vomiting and even falls. Patients find this symptom particularly distressing as the dizziness occurs suddenly and without warning. It can last for hours or even days.</p>

Patients take them to combat the acute attack of dizziness and against the nausea/vomiting. We recommend that patients carry the tablets with them at all times so that they can react immediately in the event of a sudden attack.</p>

<p>In addition to acute treatment, Ménière's disease therapy also aims to reduce the number of attacks that occur. Here, too, we can use various medical agents, such as betahistine. With the right dose, customised to the patient, the occurrence of symptoms can be significantly reduced. The medication does not have to be taken at a high dose on a permanent basis: After six months to a year, we can try to slowly reduce the dose.</p>
<p>Intratympanic corticosteroid therapy with the anti-inflammatory cortisone is also suitable as a preventive measure. Under local anaesthetic, we inject the medication through the eardrum into the middle ear. This allows it to reach the inner ear, where it can exert its anti-inflammatory and vasoconstrictor effects.</p>
