			<h3>How a nasal spray works</h3>
<p>A nasal spray works in different ways depending on the type and active ingredient. The most common variants are decongestant, moisturising and anti-allergic nasal sprays:</p>
<h5>Decongestant nasal sprays (alpha sympathomimetics)</h5>
<p>They contain active ingredients such as xylometazoline or oxymetazoline, which can damage the blood vessels in the <a href="https://de.wikipedia.org/wiki/Nasenschleimhaut" target="_blank" rel="noopener">Nasal mucosa</a> narrow. This causes the mucous membrane to swell, the nasal passages become clear again and breathing becomes easier. These sprays take effect within a few minutes and usually last for several hours.</p>
<h5>Moisturising nasal sprays</h5>
<p>These sprays use saline or hypertonic salt solutions to keep the mucous membranes moisturised. This is particularly helpful in dry air or with mild infections. They work purely mechanically by loosening mucus and supporting the natural cleansing function of the nose. As they do not contain any decongestants, you can use them long-term without hesitation.</p>
<h5>Cortisone-containing or anti-allergic nasal sprays</h5>
<p>These sprays are mainly used for allergies or chronic nasal complaints. Preparations containing cortisone reduce inflammation and swelling over a longer period of time, but only take effect after a few days. Anti-allergic nasal sprays often contain antihistamines or mast cell stabilisers, which block allergic reactions directly in the nasal mucosa.</p>
<p>Depending on the symptoms, a different nasal spray may therefore be useful.</p>

			<h3>What happens if the nasal spray doesn't work?</h3>
<p>If a nasal spray no longer works as usual, this can have various causes. There are different reasons depending on the type of spray and the individual symptoms:</p>
<h5>Familiarisation effect with decongestant nasal sprays</h5>
<p>You should only use decongestant nasal sprays for a few days. Prolonged use can lead to your nasal mucous membranes becoming accustomed to the spray and no longer decongest properly without it (<a href="/en/what-are-the-consequences-of-nasal-spray-addiction/">Nasal spray addiction</a>). This can cause drug-induced rhinitis - the nose remains permanently blocked, even if there is no longer a cold.</p>
<h5>Incorrect application</h5>
<p>A nasal spray only develops its full effect when used correctly. Mistakes such as inserting the spray head incorrectly, pulling it up too quickly or failing to inhale briefly while spraying can result in the active ingredient not being optimally distributed.</p>
<h5>Active ingredient too weak or unsuitable</h5>
<p>Not every nasal spray helps with every cause of a blocked nose. For example, a mild seawater spray can moisturise a dry nose, but cannot relieve acute swelling. Conversely, decongestant sprays also do not help against allergic symptoms.</p>

			<p>A chronic <a href="/en/treating-chronic-sinusitis/">Sinusitis</a> is caused by prolonged or recurrent inflammation of the mucous membranes in the pneumatisation spaces. As a result of this inflammation, the mucous membranes remain permanently swollen, which is often accompanied by increased mucus production. As the mucus cannot drain properly, nasal congestion is further exacerbated.</p>
<p>Decongestant nasal sprays can provide short-term relief, but do not resolve the underlying inflammation. Long-term treatment therefore requires the use of nasal sprays containing cortisone and regular nasal rinsing with saline solution. In the case of a bacterial infection, the administration of antibiotics is also useful. In particularly stubborn cases, surgery may be necessary to permanently improve the ventilation of the sinuses.</p>

<p>Nasal sprays or tablets containing cortisone are primarily used for treatment in order to shrink the polyps and reduce inflammation of the mucous membranes. For large <a href="/en/remove-nasal-polyps/">Nasal polyps</a> or severe symptoms, surgical removal may be necessary.</p>

<p>Although decongestant nasal sprays can provide short-term relief, they do not tackle the actual cause of the symptoms. Sustainable treatment therefore includes the use of anti-allergic nasal sprays with antihistamines or cortisone, the targeted avoidance of allergy triggers and, if necessary, hyposensitisation in order to reduce the hypersensitivity of the immune system in the long term.</p>

<p>As the cause is anatomical, conventional nasal sprays only have a limited effect and cannot eliminate the constriction. In mild cases, moisturising nasal sprays or nasal rinses help to keep the mucous membranes supple. In severe cases, however, surgical straightening of the nasal septum (<a href="/en/nasal-septum-surgery-frankfurt/">Septoplasty</a>) may be necessary.</p>

As the swelling mechanically narrows the nasal passages, conventional nasal sprays often do not help sufficiently.</p>
<p>Treatment options include cortisone sprays or special procedures such as nasal concha reduction (<a href="/en/laserconchotomy-frankfurt/">Laser conchotomy</a>) in question.</p>

Depending on the size of the perforation, symptoms such as obstructed nasal breathing, crust formation, frequent nosebleeds or a whistling breathing sound may occur.</p>
<p>As conventional nasal sprays cannot alleviate the symptoms in the long term, treatment initially consists of moisturising the nasal mucosa with nourishing nasal sprays or ointments. If the symptoms are severe, surgical reconstruction of the nasal septum may be necessary.</p>

<p>Sinusitis is an inflammation of the mucous membranes in the paranasal sinuses. The sinuses are air-filled cavities in the skull that are connected to the nasal cavity. Their main function is to humidify, warm and filter the air that is inhaled. 			<h3>Nasal turbinate hyperplasia: How we can treat enlarged turbinates</h3>
<p>Conservative therapy is possible: Depending on the initial situation or cause of the turbinate hyperplasia, we recommend avoiding the trigger (weaning off nasal spray, avoiding allergens, treating the allergy, etc.). Treatment with cortisone can also help.</p>
<p>If this conservative therapy does not lead to success, surgical treatment is advisable. <a href="/en/laserconchotomy-frankfurt/">Laser conchotomy</a> we can reduce the size of your turbinates using a special laser.</p>
<p>Before the laser treatment, we insert cotton swabs soaked in an anaesthetic into your nose for about half an hour. No injection is therefore necessary for the anaesthetic. We can then use the laser to painlessly reduce the size of your nasal swelling in just a few minutes. No tamponade is required after the procedure. You can go home afterwards.</p>
<p>Laser conchotomy is a low-risk and effective method of restoring your nasal swelling to a normal size. This significantly improves your nasal breathing.</p>

<p>The nose is divided into the left and right nasal cavities. The two are separated by the nasal septum. Nasal polyps are also frequently associated with bronchial asthma, (chronic) bronchitis and an intolerance to the painkiller acetylsalicylic acid (ASA). Patients suffering from cystic fibrosis or Kartagener's syndrome also often develop nasal polyps.</p>

The cortisone spray must be used regularly over several months. It is also possible to administer cortisone in tablet form.</p>

			<h5>Removing nasal polyps with surgery</h5>
<p>If drug therapy does not bring sufficient success or no real or long-lasting improvement, we can remove the nasal polyps surgically. This also applies if the polyps are simply too large. The polyps are removed using microscopic/endoscopic minimally invasive surgery. <a href="/en/sinus-surgery-frankfurt/">Paranasal sinus surgery</a>. We choose the nasal openings as the access route for this. With microscopic and endoscopic support, we locate the polyps in the paranasal sinuses and remove them using special instruments. We take great care to protect the surrounding important structures (eyes, brain and blood vessels). The aim of the operation is to permanently improve drainage of the sinus cavity into the nasal cavity.</p>
<p>After the nasal polyps have been removed, we recommend careful follow-up treatment. In this way, we can reduce the likelihood of the polyps reappearing. Follow-up treatment includes the use of nasal sprays containing cortisone and regular moisturising of the mucous membranes. Inhalation or a nasal douche are suitable for this.</p>

<p>A blocked nose can be treated in various ways. The therapy depends on the cause. There are basically the following options:</p>
<h5>Medication</h5>
<p>If your nose is blocked, you can use decongestant nasal drops or nasal sprays. But be careful: you should only use the medication a maximum of three times a day and for no longer than a week. Short-term use is generally not expected to have any negative consequences. However, if the drops and sprays are used for longer, the nasal mucosa can become accustomed to the medication. It then demands the medication again and again to reduce swelling. <p>If the nose is blocked for a "simple" reason and there are no physical causes such as a crooked nasal septum, home remedies can also be very helpful. A common tip is to drink plenty of water or unsweetened teas. The increased fluid intake can liquefy the thick mucus in the nose and sinuses, allowing it to be removed. It is also often recommended to eat hot and spicy food. Horseradish, ginger, chilli or mustard contribute to increased blood flow to the mucous membranes and can therefore clear the airways. It is also advisable to lie on your side when sleeping. This allows the upper nostril to decongest. This allows you to breathe better. Also make sure that your head is slightly elevated. The nose gets blocked more quickly in a flat lying position. Another tip is to humidify the room air with the help of a humidifier or an indoor fountain, especially in the winter months when the heating is running frequently and at high levels. This means that the nasal mucosa does not dry out so easily and is less irritated. Regular ventilation is also important. Oxygen can make breathing easier.</p>

			<h3>Causes of chronic sinusitis</h3>
<p>Various factors can contribute to the development of chronic sinusitis. The most common triggers include changes that prevent adequate ventilation of the nose. These can be allergic swelling or inflammation, but also anatomical changes such as a curved nasal septum, narrowing of the sinus openings, nasal polyps or cysts.</p>
<p>Other causes can be diseases such as <a href="https://flexikon.doccheck.com/de/Mukoviszidose" target="_blank" rel="noopener">Cystic fibrosis</a>diseases of the immune system or benign or malignant tumours. What most patients don't even think about is that dental inflammation can also cause chronic sinusitis. This is the case, for example, if there is inflammation of the root of the tooth. You ("this" would mean the root of the tooth. Always refers to the latter noun) can ascend into the maxillary sinus (dentogenic sinusitis) and manifest itself there.</p>

			<h3>Conventional measures to treat chronic sinusitis</h3>
<p>Whilst a short cold is annoying but not very troublesome, it is unpleasant to be unable to breathe properly through the nose in the long term. Chronic sinusitis can be treated conventionally. These include, for example</p>
<ul>
<li><strong>Nasal sprays containing cortisone</strong><br />
The medication cortisone reduces the swelling of the mucous membranes in the sinuses. The full effect of cortisone sprays usually sets in after a few days. To achieve lasting relief from the symptoms, it is often necessary to take the medication for several months. Cortisone sprays are often used for nasal polyps or to avoid surgery. The disadvantage is that the sprays can irritate or dry out the nasal mucosa, especially if used over a long period of time. This can lead to increased nosebleeds.</li>
<li><strong>Antibiotics</strong><br />
Sinusitis is usually caused by viruses. Antibiotics that are primarily effective against bacteria are therefore only useful in rare or specific cases: in the case of extremely severe symptoms, fever above 38.5 degrees and worsening symptoms, as well as in patients with chronic inflammatory lung disease or other serious underlying illnesses.</li>
<li><strong>Antihistamines</strong><br />
If the sinusitis is the result of <a href="/en/allergy-treatment-frankfurt/">Allergy</a> treatment with antihistamines can be useful.</li>
<li><strong>Conventional decongestant nasal drops/nasal sprays</strong><br />
Nasal drops and nasal sprays with active ingredients such as xylometazoline soothe the mucous membrane and reduce swelling. As a result, nasal breathing improves temporarily. This can help you sleep better at night, for example. The disadvantage of these medications is that the decongestants can also quickly have the opposite effect. This means that the nasal mucosa swells up again after just a few hours. The more often you use the medication, the stronger this effect can be (keyword: <a href="/en/what-are-the-consequences-of-nasal-spray-addiction/">Nasal spray addiction</a>). Nasal drops and nasal sprays should therefore not be used continuously for more than a few days. For this reason, they are not suitable for the long-term treatment of chronic sinusitis.</li>
<li><strong>Painkillers</strong><br />
Well-known painkillers from the pharmacy can relieve pain, but do not speed up the healing process in the case of chronic sinusitis. They should only be taken for a few days as they can cause stomach problems, for example.</li>
<li><strong>Inhalation and nasal rinsing</strong><br />
Many patients can achieve good results in the treatment of sinusitis symptoms by inhaling or rinsing with saline solutions. The swelling of the mucous membrane can be reduced and secretions can be loosened. It is also possible to heat water and inhale the pure vapour. If desired, essential oils (e.g. peppermint oil) can be added.</li>
<li><strong>Biologics</strong><br />
These are anti-inflammatory agents that are also used in the treatment of rheumatism. Since October 2019, biologics have also been approved as a form of treatment for severe chronic sinusitis.</li>
<li><strong>Home remedies and alternative healing methods</strong><br />
Patients can support the treatment of sinusitis with simple measures: These include an increased fluid intake to liquefy the nasal secretions. Many patients also find exposure to red light particularly beneficial. Another tip is to put your head in a higher position when sleeping. This makes it easier to breathe through the nose. Some patients swear by taking zinc. Alternative approaches such as acupuncture or osteopathy can alleviate accompanying symptoms.</li>
