The name botulinum refers to a protein that is produced biologically and has been used in a wide variety of medical applications since the 1980s. Relatively new is the application of Botulinum toxin (Botox) as a preparation for treating annoying expression lines, such as frown lines or laugh lines around the eyes. Botulinum was first authorised for the treatment of wrinkles in Germany in 2006, and has been authorised for other clinical applications since 1993. The safety of the active ingredient has been proven in a large number of different studies.

The mode of action of Botox

Due to the constant activity of the mimic muscles, wrinkles develop in the overlying skin over the course of a lifetime, for example frown lines above the nose, forehead wrinkles or laugh lines at the corners of the eyes (known as crow's feet). Botulinum inhibits muscle contraction by blocking the necessary messenger substance. Botulinum is injected into the affected muscles with a very fine needle, where it blocks the release of the messenger substance that triggers the contraction and thus inhibits the transmission of excitation from the nerve cell to the muscle. The muscle relaxes and the wrinkles in the overlying skin are smoothed or disappear completely.

Where is Botox used

Due to the excellent muscle relaxation, Botox works particularly well on wrinkles caused by frequent, mostly unconscious muscle tension, such as forehead wrinkles (often caused by strained concentration) or wrinkles caused by facial expressions (laugh lines). The classic area of application for botulinum is wrinkles in the upper third of the face, i.e. around the eyes and on the forehead. However, botulinum can also smooth out annoying wrinkles next to the bridge of the nose (known as "bunny lines") or cobblestone-like dimples on the chin. Recently, strongly protruding muscle strands on the neck and the nasolabial fold have also been increasingly treated with the active ingredient botulinum.

Procedure for wrinkle treatment with Botox

In a detailed consultation at our practice in Frankfurt, your doctor will work with you to determine which region should be treated and to what extent. After disinfecting the skin and marking the areas to be treated, the injection is virtually painless. In a few cases, the only things you will see after a botulinum injection are the small injection sites and, in rare cases, tiny bruises. These changes can be easily covered up if desired and disappear completely after a few days at the latest.

How does botulinum toxin A work against wrinkles?

BoNTA, also known as botulinum toxin A, affects the connection between nerves and muscles. It works by reducing the release of the neurotransmitter acetylcholine. This neurotransmitter is responsible for the transmission of nerve impulses, which in turn trigger muscle tension. If a muscle remains tense for a long period of time, e.g. when concentrating on looking, wrinkles can develop. How quickly do the results of wrinkle treatment with botulinum become visible?

In most cases, the first results of BoNTA injections are visible after just two to four days. The maximum effect occurs at the latest two weeks after treatment.

How long do the results of a botulinum treatment last?

Please note that treatment with botulinum is not a permanent solution to wrinkles. The muscle-relaxing effect wears off after around three to six months. How quickly and to what extent the effect wears off depends on the region treated, the dosage and your individual facial expressions. However, you can repeat the treatment in our practice in Frankfurt without hesitation.

Incidentally, experience has shown that the intervals between treatments become longer and longer. This is due to the fact that you get used to certain mimic movements.

Does botulinum lead to paralysis of the muscles in the face?

The claim that botulinum paralyses the muscles is misinformation. When BoNTA is injected, the treated facial muscles relax. This relaxation smoothes the expression lines.

Do I get a rigid, expressionless face after the injection?

Many patients who are interested in wrinkle treatment with botulinum are reluctant to have it. The reason for this is that they fear they will have an unnatural, expressionless face after the treatment. They are worried that they will no longer be able to move certain parts of their face.

Such an effect is possible in principle - but only if too high a dose is injected or the patient requests it. Therefore, not every treatment with BoNTA automatically leads to a rigid facial expression.

The result depends on the dosage adapted to the individual anatomy, muscle strength and facial expressions. Of course, the final result also depends on the individual skills of the treating doctor. If he has the appropriate anatomical and aesthetic knowledge and sufficient experience - such as Dr Fischer - you will also achieve the desired result.

For whom is treatment with botulinum toxin not suitable?

As a rule, wrinkle treatments with BoNTA are gentle and well tolerated. Nevertheless, there are reasons that speak against treatment with this active ingredient. Treatment is not advisable in the case of intolerance to components of the active ingredient. Injections should also not be carried out during pregnancy and breastfeeding.

If you are taking blood-clotting medication, you should also be careful and discuss any planned treatment with us. You have an increased risk of haematoma.
